User documentation for all Emergency Medical > shell tools. Emergency Medical > shell is a free, browser-based toolkit built for EMTs, paramedics, and ALS/BLS crews who need to spend less time typing and more time with patients. No login is required to get started: fill out the form, generate your narrative, and copy it directly into your ePCR.
It generates professional patient care narratives from structured form entries, validates reports against Illinois NEMSIS v3.5.0 standards, and bundles clinical reference tools — including a drug reference, medical calculators, and a hospital notification builder — in one place. Everything runs locally in your browser, so your patient data stays with you.
This application is designed with NEMSIS (National EMS Information System) standards in mind. NEMSIS is the national repository for EMS data that defines standardized data elements used by EMS agencies across the United States.
The validation system follows Illinois NEMSIS v3.5.0 specifications, including:
The form captures data aligned with NEMSIS element categories: eResponse (unit identifiers, care level, response mode), ePatient (demographics and SAMPLE history), eSituation (chief complaint, dispatch type), eExam (physical examination findings), eVitals (vital signs), eProcedures (interventions), and eDisposition (transport and destination).
This application uses deterministic, rule-based logic to generate patient care narratives — not artificial intelligence or machine learning. Identical inputs always produce identical outputs. No information is hallucinated or invented; every phrase in the narrative traces back to a specific form field. Generation happens locally in your browser with no internet connection required.
The narrative generator uses deterministic cycling algorithms for phrase variation (to avoid repetition) while maintaining complete predictability.
The application supports multiple narrative output formats:
